Conviva Care Center, with the support of the Coconut Creek community, celebrated the grand opening of its new location on September 9 with a ribbon cutting and a tour.
Coconut Creek commission members, the Coral Springs Coconut Creek Regional Chamber of Commerce, and community leaders attended the event.
“This is a much-needed medical center in Coconut Creek that offers premium concierge care,” said Mayor Sandra Welch.
Conviva Care Centers has 135 locations in Texas and Florida. It is a subsidiary of Humana Inc. and part of Humana’s Primary Care Organization.
According to Dr. Evan Jacobs, a specialist in cardiovascular disease, the goal of Conviva is not to process as many patients as possible but to provide the highest quality preventative care so patients recover without requiring more extensive care.
The center’s doctors spend more time listening to patients and helping them achieve improved health, ensuring a swift recovery.
“Our goal is to keep people out of the hospital,” said Jacobs.
Dr. Isabel Souffront, a family medicine specialist and the medical director for Conviva, said, “Our goal is to provide the best care we can for patients, and we are doing that here.”
Conviva’s Coconut Creek location offers ten examination rooms, four providers in its primary care area, six examination rooms, and eight providers in the specialty area. It provides specialty care in cardiology, nephrology, and podiatry.
According to Conviva, they help patients manage heart disease and COPD by providing personalized long-term care plans. Individuals can have blood drawn at an on-site lab or talk with care teams about fitness programs. It also offers a screening program for conditions such as heart failure, diabetic neuropathy, coronary artery disease, and others.
Conviva is open from 8 a.m. to 5 p.m., Monday through Friday at 4828 Coconut Creek Parkway. They also have a location in Margate at 5643 NW 29th St.
